JOB SUMMARY:

Assists supervisor with coordination of Partners In Care Program and HMC pediatric patient care; Represents the organization with outside agencies such as Children’s Medical Services (CMS) as needed; Provides direct patient care and external/internal education as needed to ensure optimal care and pediatric staff competency.

 

ESSENTIAL JOB RESPONSIBILITIES:

Include the following.  Other duties may be assigned.

·         Represents the organization, collaborating with outside agencies such as CMS during the active or current periods of the CMS-PIC service agreement - signed between CMSN and HMC.

·         Participates in the medical management of pediatric patients with life-limiting conditions to effectively manage the Plan of Care (POC) for each pediatric patient in conjunction with pediatric and palliative care physicians in hospitals and other healthcare settings.

·         Performs evaluation and assessment of pediatric patients as needed; utilizes expert skills in history procurement, physical examinations, initiation and interpretations of tests, analysis of data and the implementation and evaluation of the POC.

·         Participates in after hours/on-call duties related to HMC’s Pediatric Services.

·         Formulates appropriate differential diagnoses and identifies actual or potential health care needs.

·         Develops treatment plans in conjunction with the Pediatric Program Medical Director, and with each patient’s primary care physician.  Works with other related clinicians (including CMS clinicians, when it is related to pediatric referrals, patient emergencies and other issues from CMS).

·         Maximizes the health potential of the individual; developing, implementing and maintaining an individualized and holistic POC for each patient that is realistic and achievable towards outcomes of the pediatric patient and/or family comfort and support in each instance of a life limiting illness.

·         Utilizes a holistic approach to assess each patient’s need in an on-going and timely manner, intervening appropriately and accurately based on the patient’s POC as appropriate (or when approved by the CMS clinical team, whenever it relates to CMS-PIC patients being admitted) to HMC’s Pediatric Palliative Care Program.

·         Educates referral sources as well as pediatric clinical staff on current CMS-PIC procedures when and where applicable.

·         Assists supervisor with scheduling and facilitation of regularly scheduled interdisciplinary team (IDT) meetings; provides input related to each pediatric patient’s hospice care or palliative care.

·         Provides appropriate and pertinent patient education to the pediatric patient, caregiver, and/or family according to the POC.

·         Documents and communicates all required information in accordance with agency policies and procedures, accreditation, and all federal and state regulatory requirements.

·         Assists with managing HMC’s pediatric services in a fiscally responsible manner; operates within budgetary guidelines for the Pediatric Palliative Program.

·         Enhances practice and profession by consistently reading appropriate pediatric professional journals, fulfilling CEU’s, CE’s and other requirements (including pediatric certifications – through nationally recognized organizations).

·         Maintains all Florida State licensing requirements and standards of practice or a registered nurse.

·         Collaborates with Quality Improvement team as directed by supervisor.

·         Assists supervisor in collaborative efforts with Human Resources on all employment related risk areas, disciplinary measures, hiring practices and terminations of pediatric care staff as indicated.

·         Ensures accurate documentation and coding for billing for Pediatric Palliative Care Services and/or Pediatric Hospice Care visits.

 

QUALIFICATION REQUIREMENTS:

·     Bachelor’s Degree in Nursing required, with a major part of the nursing experience in PICU or other pediatric healthcare settings.

·     Master’s degree in Nursing preferred.

·     Current RN license to practice professional nursing in the State of Florida required.

·     Current CPR required. 

·     Certification in pediatric care by a recognized national organization already achieved or will be planned within 3 years of employment as the Assistant Pediatric Palliative Care Program Manager

·     Three years of experience with pediatric hospice and/ or pediatric palliative care patients preferred. Minimum of five years nursing experience in inpatient and outpatient settings. 

·     Experience in an acute care setting very desirable.

·     Minimum of three years supervisory experience preferred.

·     Able to interact with community representatives, market the pediatric services to various groups as

          needed. Improve or increase the pediatric referral process to the Pediatric Palliative Care Services.

·     Must have a Valid Florida Drivers License and maintain current auto insurance coverage and have access to a reliable automobile.

·     Skills in fostering positive interpersonal relationships.  Ability to accept and embrace other cultures and religious standards without imposing personal beliefs.  Ability to cope with high levels of stress.

·        Demonstrated ability to multi-task, handling caseload, stressful situations, and providing caring, compassionate, customer service throughout.

·        Ability to operate a facsimile, copy machines, computers and other common office machines, with working knowledge of MS Word, Outlook, Excel and other software appropriate to position.

·        Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures and governmental regulations, as well as documents such as safety rules, operating and maintenance instructions, and procedure manuals.
