Location: Positions available in Richmond area, Tidewater, NoVA, and Roanoke regions - Remote, work from home. Virginia residency required. Some local travel required.

This position supports our LTSS (Long Term Services and Support) members with behavioral health issues. You will W@H, via telephone, with occasional field visits, to our members where they live (mileage reimbursed).
Responsible for performing telephonic case management within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs.

Primary responsibilities include, but are not limited to:
  • Use appropriate screening criteria, knowledge and clinical judgment to assess member needs.
  • Conduct assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.
  • Monitor and evaluate effectiveness of care plan and modifies plan as needed.
  • Support member access to appropriate quality and cost effective care.
  • Coordinate with internal and external resources to meet identified needs of the members and collaborates with providers.
  • Virginia residency required.
  • BA/BS (in a health related field preferred); minimum of 3 years of experience working directly with people related to behavioral health or related community based organizations; or any combination of education and experience, which would provide an equivalent background.
  • Requires a current, unrestricted LCSW, LPC or RN license in applicable state required. Masters in Health preferred. May require state-specified certification based on state law and/or contract.
  • Experience working with community resource groups preferred.
  • Knowledge of Medicaid, managed care, and/or long term care programs preferred.
  • Computer skills: MS Office (Word, Excel, Outlook)
  • Experience working with community resource groups preferred.
  • Occasional local travel required (mileage and tolls reimbursed).
  • Knowledge of ICD-10 codes, medical terminology and coordination of benefits preferred.
