Behavioral Health Specialist

 

A full-time, work-from-home position for a RN, BSN who will serve in a complex case management role for persons with dominating behavioral health conditions. You will provide behavioral health resource support to a Care Team for members enrolled in a disease management program, focusing on members who are identified as high risk with behavioral co-morbidity.

Evaluate identified membersand develops an individualized comprehensive plan of care.

Implement the comprehensive plan of care.Build relationships and assists members and families in understanding and carrying out treatment care plans.

Provide Coordination and assistance to members qualifying for additional benefits available

Provide consultation to the care team for members who are identified as high risk with behavioral co-morbidity.

Identify, developand maintain relationship with community, health and welfareagencies andstate groups that are involved in care of vulnerable populations and are resources to the population in the program.

Minimum Job Qualifications: 

Education/Training

 

-     BSN, RN with specific training in behavioral counseling (no Rx privilege needed)

-          3+ years recent clinical experience in behavioral health or psychiatric field (prefer disease management experience)

-          Current unrestricted LMHC in TX

 

 

Specialized Knowledge/Skills:

 

-          Thorough knowledge of behavioral health issues and the application to case management including complex case management

-          Understanding of family and group dynamics

-          Familiarity with change behavior techniques

-          Knowledge of government and private organizations, resourcesin the community, laws, regulations and policies which govern clinical social work

-          Skill in establishing rapport with members, families, and local resources (medical, social, civic, legal and religious organizations

-          Excellent written and verbal communication skills

-          Computer skills to include Windows environment (preferably Word and Excel)

-          Preferably a member of the local community

-          Current CPR certification

-          Current PPD

-          Current valid driver’s license and required level of automobile liability insurance coverage.

-          Preferably bi-lingual

 

 

Working Conditions:

·         Environment – Independent, home environment with daytime travel to assigned demographicoperational areas and facilities. Must have access to high-speed internet connection (e.g. DSL, Cable) and be able to comply with Work at Home Agent Requirements.

Travel: 

·         Willingness and ability to travel to assigned operational areas/facilities. Reliable mode of transportation required.
