Bilingual Care Manager, Part-time (20 hours)

Worcester Region

 

 

Caregiver Homes is a community-based care option for nursing home-eligible seniors and disabled adults.  The program provides nursing assessment and Care Management Team support and also pays family and non-family caregivers to provide care at home – offering an alternative to traditional long-term care options.  We continue to grow and have a part-time opportunity in our Worcester region for a:  Care Manager: part-time(20 hours).  Fluency in English and Spanish is required.

 

The Care Manager will work collaboratively throughout the Worcester region ( Hardwick, Southbridge, Uxbridge, Sterling, etc.) as part of a case management team to assess clients, establish and implement a plan of care, conduct regularly scheduled homes visits, and oversee client care. The team credentials caregivers and their homes, and provides ongoing support and training.

 

Qualifications:

·         Minimum of two years experience in case management, care planning and assessment for elders/disabled adults in a health care, long-term care, social service or community setting.

·         Candidate must have a bachelor’s degree and be licensed in social work or nursing, or an equivalent combination of education and experience in these or related fields. 

·         Ability to speak, read and write fluently in both Spanish and English is required.

Skills/Knowledge Required:

 

·         Knowledge of case management systems, community service and delivery systems for aging population required. 

 

·         Strong clinical and communication skills are essential with competent use of technology applications. 

 

·         Regular car travel within assigned regional geography is required.
