JOB SUMMARY


 


The Blended Service Coordinator provides highly personalized services to a caseload of severely and persistently mentally ill consumers for the purpose of maximinizing their stability and integration into community life via Recovery Oriented Services and Single Point of Accountability (SPA) approaches and tools.


 


JOB SPECIFICATIONS


 


Master's degree in Human Services field and one year experience in Mental Health services required OR Bachelor's degree in Human Services field and three years post Bachelor's experience, one of which must be in Service Coordination.  Proven competency in implemenation of SPA and Recovery Oriented Services is preferred.


Must have computer experience with skills in Word, Excel and the Internet.


 


Valid driver's license and insurability based on Agency policy is required.


 


The position requires the following knowledge, abilities and skills:


 


* Considerable knowledge of common mental disabilities and their obvious symptoms.


* Considerable knowledge of individual and group behavior and the interrelationships between social, economic, psychological and physiological factors.


* Knowledge of Service Coordination forms and techniques.


* Knowledge of social service systems, type of services commonly available and how to access them.


* Ability to organize and prioritize a variety of work in a manner which ensures accurate and timely completion in an environment of changing conditions and situations.


* Skill in assessing consumer needs, developing service plans, and accessing, coordinating and monitoring services.


 


RESPONSIBILITIES


 


* Implementation of Core Affirmative SPA responsibilities.


* Develop comprehensive consumer Recovery-Oriented Service Plans.


* Assist consumers in obtaining benefits and monitors FFP status through monthly checks on Medical Assistance status.


* Assist consumer in developing realistic goals.


* Maintains a minimal of monthly face-to-face contact with consumers or provides documentation of attempts.


* Participates in a system of Blended Service Coordination emergency after-hours coverage and on-site intervention.


* Provide financial assistance to consumers in emergency and priority situations with supervisory approval through the use of contingency funds.


 


This position is a temporary position (approximately 6 months) with the possibility of becoming a full time position.
