Care Coordinator / 1203
Position Description:
Provides assertive care coordination and outreach services to consumers referred to the SMI programs under the direct supervision of a qualified case manager.
Essential Functions: 1. Identify and provide non-clinical community based services to consumers enrolled in the SMI program. (15%) 2. Coordinates with Peer Specialist to engage member and assure member input into all aspects of the Care Plan or Individual Recovery Plan (IRP) (10%) 3. Identify and arrange for Community Resources that support SMI members that may include: • Activities of Daily Living (ADL) • Transportation • Entitlements • Housing • Clothing • Dental Services • Vision Services • Literacy Services • Cultural and Linguistic Services • Legal and Advocacy Services (10%)
4. Engages with the consumer in side by side modeling of how to enjoy various leisure-time activities. (10%) 5. Engages in pre-planning meeting with the consumer to help them understand and prepare for the recovery planning process, and begin thinking about his/her goals before involving the psychiatrist and other clinical recovery team members. (10%) 6. Initiates a crisis response plan using consumer’s self report of intervention preferences. (5%) 7. Provide members with information about the Case Management process and services. (15%) 8. Participates as a member of the case management team providing supportive services that do not require the clinical judgment of a licensed clinical case manager, licensed clinical supervisor and/or psychiatrist. (20%) 9. Participates in 24/7 on call rotation
Education: o Bachelor’s degree with a minimum of twelve (12) semester credit hours in courses such as counseling, criminal justice, human services, psychology, social work, social welfare, sociology or other behavioral sciences; AND a) one and one-half (1½) years of specialized experience or b) at least 40 hours of continuing education in human/social services subjects. The continuing education may consist of seminars, conferences, adult education courses, certification coursework, employer training, or similar educational activities.
Experience: • Minimum of two (2) years experience in direct inpatient and outpatient mental health clinical care preferably in behavioral health field. • Minimum of two (2) years utilization management and/or case management experience with quality improvement experience
