1. Provides telephonic follow up with the member for case management services after discharge from the facility or communication with the health plan's case managers for follow up needs with documentation in the Care Coordination form.

2. Works with member, physician other Health care providers and the car managers to meet the member's individual needs based on protocols and referrals. Calls the identified member to offer a review of the CareContact Disease Management program. Supports the member education goals regarding their specifice chronic condition based on the Plan of Care. Informs the member how to access the Care Contact program resources. Assists in the arrangement of Provider follow-up appointments. Mails approved educational and informational materials. Assists in the performance of "Welcome Home" calls to members post discharge as directed by the Care Managers. Supports the monioring and evaluation of interventions as directed by the Care Manager in the Plan of Care by periodic telephonic contact with members based upon data received pertinent to members chronic conditions or other risk triggers, ie; ED visits, IP acute stay, referral for out patient cardiac procedure.

3. Identifies themselves as agents of the Health Plan when conducting telephonic facility utilization reviews and/or CareContact communications. Internal communications will be conducted in a professional manner. They must remain objective in their review and not participate in any way in the delivery of care to the patient and comply with all departmental policies and procedures, participate in departmental and company in-services as appropriate, perform other duties as assigneed by the Care Managers, and document as required according to CareContact policy and procedure.
4. Administers tasks as detailed in the member specific Plan of Care, under the direction of the Care Managers. 5. Complies with Customer Service expectations as applicable to the department and Health Plan. 6. Recognizes and adheres to the scope/limitations of job requirements, which may include the task of a CareContact screener and/or auditor for the Health Risk Assessment screening tasks as sent to Care Managers or Care Management Assistants.
