My client is a specialty Cancer hospital located midway between Chicago and Milwaukee.

Their experienced and dedicated cancer experts work together in one location, sharing their knowledge, coordinating treatments and, ultimately, providing tailored Cancer and oncology treatment to you.

Individual patient teams of may include medical, surgical and radiation oncologists; radiologists, registered nurses, care managers, registered dietitians, naturopathic clinicians, mind-body therapists, pain management practitioners, physical therapists, and pastoral care providers.

Their staff also includes a number of specialists, such as a female gynecologic oncologist, hematologist/oncologist, gastroenterologist, and reconstructive surgeon.

This position is responsible for the coordination of all systems/services required for an organized, multidisciplinary team approach and assures quality, cost-efficient care for an oncology patient caseload.  The Care Manager functions as advocate and contact person for patient, family, healthcare team members, employees and outside agencies to assure continuity of care and optimal clinical and resource outcomes. Participation in an on-call rotation will be required based on departmental need.

Minimum Qualifications:

·         BSN in Nursing preferred

·         Current Illinois RN Licensure

·         3 years clinical oncology experience preferred

·         1-year experience in Care Management and/or similar role

·         Strong organizational and communication skills

PHONE TRIAGE OR CLINIC EXPERIENCE STRONGLY PREFERRED
