Care Manager-Weekends

Facility: Corporate Services

Department:

Part time Weekend position. Management for the assigned inpatient Care Management population. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient throughout; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Actively supports Care Management plans and processes to measure, assess and improve quality, patient outcomes and care management metrics. Provides the professional clinical component of the impatient care management model to ensure that department services support quality patient care and safety while coordination patient throughout. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards. 77483
Job Requirements Required
  • Graduate of an accredited school of nursing
  • Bachelor of Science in nursing required
  • Current License in the state of employment
  • 2-3 years acute care hospital setting
  • BLS (CPR) at hire date
  • Strong Verbal and Written Communication and facilitation skills
  • Strong assessment Skills, Time management skills, critical thinking skills
  • Advanced Computer Skills
  • Must have ability to interact effectively with a variety of people and situations at all levels of the organization
  • Team work is essential
  • Portrays a positive professional public image at all times
  • Demonstrates ethical behavior and provides confidentiality in all care managemmen interactions.
  • Provides care management education to peers and customers' participates in continuing educational activities
  • Exhibits behavior and adopts practices to ensure optimal risk management and financial stability


  • Preferred
  • Care management or Utilization Management experience
  • Certification in Case Management
  • Knowledge of InterQual criteria
