What you’ll do:

§     Facilitate patients’ access to quality, cost-effective health care.

§     Develop and monitor patient-specific goals and interventions in collaboration with attending physicians.

§     Provide reports on findings, recommendations, and outcomes.

What you bring:

At least four years of clinical experience and either:

§     A Bachelor’s degree in a healthcare field, or

§     Current, active, and unrestricted Alaska RN licensure.

CCM is a plus.  The incumbent will telecommute upon successful completion of initial orientation.
