Primary Care Coordinator
[$25,000 starting salary ] Qualifications: Bachelor's Degree; 2 years post degree experience; 

Position available is immediate and full-time.
Qualified applicant should possess the following skills:

• Utilize treatment team model
• Responsibilities include treatment planning and all other required documentation
• Case Management, crisis intervention; & facilitating psycho educational groups
• Confidentiality, People skills, Time  Management, Quality Focus, Written & Verbal Communication, Motivating  Others, Building Relationships.
