Description: 

We are currently seeking a Case Manager Registered Nurse for our Owensboro, Kentucky Amedisys Home Health agency. This position requires previous home health care experience.

The Case Manager Registered Nurse will be responsible for the care coordination and provision of quality home nursing care and maintaining the highest standards of care and ensuring compliance with all policies, procedures and regulatory requirements.

Requirements:

1. Current and unencumbered state license to practice as a Registered nurse.

2. At least one year experience as a Registered Nurse.  One of those years must consist of full-time experience in providing direct patient care in the home health setting.  One year supervisory/management experience preferred.

3. Current CPR certification

4. Able to assess patient status and identify requirements relative to age specific needs.

5. Excellent verbal and written communication skills.
