

What We Do:
HeartShare Human Services of New York was founded in 1914 as an after-care agency for young adults aging out of orphanages. HeartShare helped them face the harsh realities of life-on their own-from finding a home to getting a job. As society’s needs have changed, so has HeartShare.
Today, HeartShare serves 17,000 children, adults, and families in more than 60 programs. Its services for individuals with developmental disabilities include: early childhood services for children ages 3-5, Universal Pre-Kindergarten for children age 4, a school-age program for children ages 5-14 with autism spectrum disorders and mental retardation, recreational opportunities for teens and adults, adult day services, residential services for adults, family support services, case management, and health and mental health services.
Additionally, HeartShare offers foster care and adoption services, after-school youth programs, family counseling, services for those affected by HIV/AIDS, and energy assistance programs.
Its mission is to nurture and support, with dignity and respect, children, adults and families in order to expand opportunities and enhance lives. Approximately 90% of all revenue goes directly to program services. HeartShare is accredited by the Council on Accreditation of Services for Families and Children and meets the standards of the Education and Research Foundation for the Better Business Bureau.

CASE PLANNER
Job Description:
The Case Planner is Responsible for assessing child and family needs and implementing preventive services in an effort to avert, when possible, foster care placement, and disruption of the family unit. Case will evaluate the social, developmental, and resource needs of children and families; develop and implement individualized service plans, and when necessary, provide protective and placement services for dependent or neglected children. Work includes counseling families toward resolving individual and family problems and utilizing agency and community resources to assist families in achieving and maintaining self-sufficiency and independence. Work is performed under the supervision of an administrative superior, usually the Case Work Supervisor. Close supervision is received in the form of review of casework plans, one-on-one supervision, and frequent casework conferences.
