Nurse Practitioner/ Physician Assistant
POSITION SUMMARY:
We invite you to choose a career with our growing Physicians Home Visits team! As a team of Board Certified Physicians, Nurse Practitioners, and Physicians’ Assistants, we work together to provide primary care services for seniors in their homes. Our goal is to provide seniors with the highest quality medical services in the greater Triad area, emphasizing wellness and management of chronic diseases. Each practitioner is partnered with a certified medical assistant to ensure exceptional, quality care for each patient. Candidates with home health, assisted living, urgent care, and/or ICU experience are encouraged to apply. We offer a regular schedule (no call or weekends) and a competitive salary and benefits package.
MAJOR DUTIES AND RESPONSIBILITIES:
· Examines patient, compiles patient medical data, and results of examination.
· Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and healthcare needs.
· Collaborates with Physician and other health professionals to prepare comprehensive, client-specific plans of care.
· Performs therapeutic procedures, such as injections, immunizations, and wound care, and managing infection.
· Instructs and counsels patients regarding compliance with prescribed therapeutic regimens and health maintenance.
· Refers patients requiring specialty services to specialist, other health professionals, or appropriate community resources.
· Maintains medical records.
· Assists in the development, implementation and revision of policies affecting the medical practice and quality of health care delivered.
· Directs duties of certified medical assistant.
· Maintain professional and technical knowledge by attending educational workshops, establishing personal networks, and participating in professional societies.
PREFERRED SKILLS AND EXPERIENCE:
· Customer and personal service knowledge including needs assessment, meeting quality standards for services and evaluation of customer satisfaction.
· Demonstrated interpersonal and communication skills.
· Must be able to develop productive and cooperative working relationships with all members of the health care team and personnel from other agencies.
· Ability to work independently and proficiently with minimal consultation.
· Competent in use of Electronic Medical Records.
· Clinical experience in home health, assisted living, and/or urgent care desired.
EDUCATION/CREDENTIALS:
· Licensed and certified to practice in North Carolina.
· Graduate of an accredited Nurse Practitioner/Physician Assistant program.
· Maintain a current insurable driver’s license.
