CLINICAL CARE REP/MA – Coordinates  with  the  patient  their  post-visit  care  plan  ordered  by  their  physician,  NP  or  PA.  Educational and resource information will be reviewed with the patient.  Hand-outs  and  appointment  cards  will  be  distributed  to  the  patient.    Ability  to  establish  and  maintain  effective  working  relationships  with  patients,  providers  and  staff  members.    Ability  to  communicate  clearly,  concisely  and  effectively  with  a  patient.    MA experience and medical terminology required.
