Duties include receiving and documenting incoming phone calls from referral sources to set up home care for clients. You will function as a facilitator between Case Managers, Discharge Planners, Client Services Managers, and Clinical Managers. The ability to cultivate positive long-term relationships with referral sources combined with sound clinical judgment is integral to success in this position. The successful candidate will be a licensed RN or LPN and have a minimum of two years of intake experience. Excellent communication and interpersonal skills required. Previous experience in community health field preferred. The candidate must be able to work Monday through Friday, 10:30 AM EST to 07:30 PM EST on site at our Parsippany, NJ office.
