Job Description

Clinical Director/Team Leader- FACT Program

As part of the Telecare community, the 50 member San Bernardino FACT program located in

the city of San Bernardino provides Forensic Assertive Community Treatment to

adults with a severe mental illness. Services are delivered primarily in the

community -- where members live -- by a multidisciplinary team with a low

staff-to-member ratio. We recently expanded our programs to work specifically

with persons with a forensic history and those who are high users of psychiatric

inpatient hospitalizations.

 

The Clinical Director/Team Leader oversees and coordinates the day-to-day functioning of the

culturally competent teams, oversees performance measures and all applicable

State regulations.  The individual ensures that each consumer receives the skills, training and

supports necessary to lead successful and satisfying lives in the setting of their

choice.

 

 

Qualifications:

 

-Master’s degree in social work, counseling, nursing or related field is required for this position                                                                                            


-Licensed as an LCSW, MFT, Ph.D, Psy.D, Certified OT, MSN or a Masters Degree and currently registered with the BBHS

-Experience supervising professional, para-professional and support staff, the incumbent must have experience working with consumers, families and advisory boards boards                                  

-An understanding of intensive case management and psychiatric rehabilitation concepts                                                                                      

-A successful track record working with, and sensitivity to, multi-cultural populations 

is highly desirable. 

-Must have a valid California driver’s license, a vehicle (approved for safety), proof of insurance and have the knowledge and ability to use public transportation
