


Affinity Health Plan, an independent, growing, not-for-profit managed care company designed specifically to serve the needs of low-income New Yorkers, needs a Clinical Pharmacist to collaborate on the development of targeted pharmacological health management programs, coordinate with other health management program activities, and consult with the Utilization Management, Quality Management, Provider Relations and other departments as needed. As part of your responsibilities, you will: 1. Conduct research and analysis and make recommendations to implement targeted pharmaceutical care and cost management programs. 2. Interface with other departments (IS and QM) on the development and maintenance of databases to support and document program activities. 3. Collaborate as a pharmaceutical care specialist with the CMO staff in other departments on the planning and implementation of total health care programs, including prior authorization and drug utilization review. 4. Collaborate with the internal on the planning and implementation of member education programs directed at select audiences in the most appropriate settings. 5. Collaborate with physicians and other prescribers by reviewing patients’ drug profiles to help safeguard against new prescriptions having unintended side effects and by working with prescribers to assure that the drugs prescribed are eligible for coverage; are low cost, high quality generic products when appropriate; and are projected to do no harm. 6. Requirements for this position include: · BA/BS in Public Health or scientific or quantitative field and PharD degree · Minimum of 3 practicing or consulting pharmacy experience · Minimum of 5 years managed care, public health or related experience · Two (2) years management experience, including project management skills, with ability to motivate internal and external collaborations to see projects through to completion · Expert level proficiency with Microsoft Applications and experience in developing and maintaining Access databases · Demonstrated proficiency with the statistical reporting of data and the ability to analyze, draw conclusions, and make recommendations from qualitative and quantitative information and data. · Excellent presentation skills and ability to communicate clearly, both verbally and in writing · Self motivated, with demonstrated ability to work independently with minimal supervision · Strong interpersonal, organizational, supervisory, skills characterized by ability to motivate, mediate, counsel and mentor others. · Ability to communicate clearly and effectively, both verbally and in writing. · Assertiveness and the ability to work with limited guidance · Bilingual English/Spanish preferred · High level of integrity as demonstrated by 1) appropriate treatment of confidential information, 2) adherence to policies, procedures, rules and regulations, 3) professional conduct in dealing with persons internal and external to the organization and 4) sensitivity towards target population.
