Clinical Social Work Case Mgr
The Clinical Social Work Case Manager under general supervision of the Principal Investigator, the Program Director, and the Manager of Lifespan Office of Research Administration, provides case management and intensive follow-up to HIV positive individuals exiting prison or drug detoxification facilities. Maintains all requested documentation to meet the requirements of the project. All essential functions performed in this position reflect the age-specific needs of the patient.
Requirements:

Is responsible for knowing and acting in accordance with the principles of the Lifespan Corporate Compliance Program and Code of Conduct. Completes a comprehensive psycho-social evaluation and resulting treatment plan for each assigned client and maintains follow-up activities, as required. Convenes and facilitates quarterly case conferences with client and representatives of agencies providing services, provides required documentation. Makes home visits within 24 hours of clients’ discharge, and, as needed, thereafter.
Provides for crisis intervention services, as needed, completion of this function may extend past usual work hours. Provides guidance and leadership with Outreach Team Member. Participates in team meetings and utilizes individual supervision appropriately.
Adheres to the National Association of Social Workers (NASW) Code of Ethics, with special recognition of the ethical, professional, and legal obligation to uphold the principle of client confidentiality.
Maintains all standards of written documentation in a timely and thorough manner. Serves on community or agency committees, as requested. Maintains quality assurance, safety, environmental, and infection control, in accordance with established hospital department policies, procedures, and objectives. Performs other related duties, as required.
Masters Degree in Social Work. Licensed, or license-eligible, to perform social work services in the state of Rhode Island. Knowledge of substance abuse programs and community support groups. Bi-lingual Spanish, a plus. Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Two years of professional social work experience with minorities, substance abuse, prison population, or related field.
