Clinician Diabetes

With the support and direction of the Program Director and the Healthways Clinical Department, the Diabetes Clinician is responsible for implementation of assigned program aspects that address objectives developed in partnership with the company and the customer. This position requires a committed individual who demonstrates a systems approach to management. The Diabetes Clinician will translate business knowledge into behaviors that produce high quality, cost-effective outcomes. This individual is able to apply program evaluation and accepted clinical research findings to secure clinical and program outcomes. He/she will build strong customer relationships, actively explore customer needs, and pursue creative solutions to satisfy those needs and monitor results on a continual basis.

Responsibilities Include:


Organizational:


  • Works with the Program Director, physicians, Resource/Utilization/Case Management, diabetes staff and other hospital staff to achieve program objectives and enhance clinical outcomes and customer satisfaction.

  • Participates in the marketing and media plans established for the Diabetes Service Line. Represents Healthways to physicians, local and national organizations, and community groups. Represents the customer, Bon Secours, in community benefit activities, at least once a quarter, to promote the customer's mission. Examples: ADA Walk, JDRF Walk, health fairs, etc.

  • Actively participates in and integrates current hospital structure, policies and procedures, order sets, and forms, as related to diabetes care.

  • Based upon customer needs, identifies and utilizes available and appropriate community services to enhance the diabetes services.

  • Schedules hours/activities based on customer needs. Clinicians will be expected to teach one night class per month on average. The five day work may be modified to include Saturdays if necessary.

  • Conducts and tracks meaningful interactions with physicians.

  • Based on program objectives, and with guidance from the Program Director and the Healthways Clinical Department, determines appropriate methods to measure, analyze, and interpret results to clinicians and administrative staff.

  • Establishes specific outcome measures for aggregate clinical diabetes populations (such as the elderly, those with specific chronic complications, pregnant, etc.)

  • Establishes a system for understanding and communicating effectively with physicians involved in diabetes care and for assessing physician satisfaction with diabetes service line.

  • Explores and communicates opportunities for role and service enhancement to Program Director.

  • Mentors other clinical staff members in achieving outcomes.

  • Clinical:


  • Assess patient's understanding of disease process; chronic complications; standards of care and current areas requiring clinical intervention via interview and self-assessment and/or medical record.

  • Determine patient's readiness for education and possible barriers to implementing self-care plan for disease management.

  • Assist patient to set self-care goals to improve disease management.

  • Provide education and implement interventions such as medical nutrition therapy, self blood glucose monitoring, medication/injectables, insulin pump therapy (if certified) to improve disease management.

  • Adjust interventions with consideration to other disease processes that affect plan of care such as, but not limited to, CVD, CAD, renal complications; developmental, physical and/or mental disabilities.

  • Instruct patient in classroom and/or individual settings (outpatient and in-hospital).

  • Demonstrate the ability to adapt education and teaching tools to the patient's level of readiness, understanding and/or literacy.

  • Teaches classes and performs consultations according to Healthways established curriculum and documents all interventions appropriately.

  • Serves as a resource to program staff, nursing, and ancillary dept. staff.

  • Ensures that all components of the program meet the Quality Assurance Standards as established by the company and the hospital.

  • Participates in Performance Improvement/Quality Assurance Advisory Committee meetings and contributes to yearly program review. Participates in other hospital team meetings as necessary to ensure hospital-wide continuity of care.

  • Assists in orientation and development of diabetes treatment team as required.

  • Keeps written documentation and data on outcomes to improve organizational and clinical systems of care.

  • Facilitates case management of complex diabetes cases through Diabetes Inpatient Clinicians, Case Managers, Social Workers, and other clinicians.

  • Facilitates the process for development and implementation of Performance Improvement Teams.

  • With the support of Healthways Clinical, modifies or develops practice guidelines, Standards of Care, and Policies and Procedures, etc.

  • Establishes a system for providing a follow-up care for target diabetes groups.

  • 18. Develops excellent physician relationships; facilitates frequent and clinically effective face-face interactions.

  • Complies with the hospital's medical record documentation standards

    Financial:


  • Explores strategies to maintain and/or reduce resource consumption; to reduce length of stay; facilitates implementation of these strategies house- wide; and to monitor results.

  • Supports physician marketing activities.

  • Participates in data collection and analysis to meet the outcome objectives.

  • Utilizes the value cycle with all customers on an ongoing basis.

  • Participates in translation of outcome data into cost management terms.

    Professional Development:


  • Maintains a state of the art approach to diabetes care and self management training.

  • Is proactive in seeking latest information about practice and theory in the area of diabetes and its complications and shares information with the diabetes team in a timely manner.

  • Networks with other Healthways Diabetes Clinicians.

  • Performs other duties as assigned.

  • Obtains and maintains CEU certification.

DESIRED QUALIFICATIONS:

  • Registered nurse or registered dietitian with a minimum of a Bachelor of Science degree with previous two years experience focused in diabetes care.

  • Current unrestricted state nursing license or CDR certification for dietitians.

  • The ability to communicate effectively with professionals in diverse disciplines.

  • Certified Diabetes Educator preferred or has qualifications to take the national exam within two years of hire and obtain credentials.
