The Health Coach is part of a multi-disciplinary team that is responsible for improving both the individual health and wellness of program participants and the overall health status of a client's population. The Health Coach is an integral part of an overall client specific Health and Care Support Team that engages and supports members with lifestyles and behaviors that put them "at risk" of developing chronic or disease conditions. The Health Coach looks to be a positive change agent in helping members alter behaviors that lead to improved health and reduction / elimination of identified risk factors. The Health Coach is part of a broader Optimal Health and Care Team that includes Nurses, Registered Dieticians and Social Workers. This Health and Care Support Team leverages each others strengths and knowledge base in addressing the specific needs of each member and improving the health status of the individual and of the client's population.
Specific responsibilities include but are not limited to:
- Contacting participants that are eligible for the program for enrollment and provides necessary coaching to reduce or eliminate high-risk behaviors
- Encouraging adoption of habits that are conducive to a higher quality of life
- Coordinating delivery of services and care with other members of a multi-disciplinary care team, including nurses
- Supporting operational aspects of the division to meet the organization's customer requirements we offer our colleagues:
- Formalized ongoing training and education, including a 2 week comprehensive training program
- Set work schedules, Monday - Friday, with limited Saturday coverage (closed Sundays and holidays)

Qualifications and Requirements:
- Bachelor's degree in health-related field required.
- Requires effective interpersonal and communication skills
- Proficiency in computer data entry and Microsoft Office
- Must be able to utilize telephone and input/retrieve computer information for extended periods of time
- Must be able to understand directions and communicate/respond to inquiries
- Licensures and Certifications as appropriate
