We offer a competitive annual base salary plus monthly incentives of $30 per hour for hours worked in excess of 30 hours per week!
Responsibilities include:
Initiates communication with case manager and other service professionals in the client’s life to coordinate services

Minimum Qualifications:
Applicants must have minimum of bachelor’s (Masters level preferred) degree in human services field plus one year of direct clinical experience. Please include month and year of previous related work experience.
