CareMore's Primary Care & Collaboration division oversees strategy, operations, and care delivery in our primary care markets (Iowa, Tennessee, Connecticut), where CareMore builds and runs capitated primary care medical groups and currently serves over 22,000 high-risk Medicaid and Medicare patients, as well as our collaboration with Emory Health System, which brings the CareMore model to over 20,000 Medicare Advantage patients in Georgia. The division also supports the development of new markets and models in the Primary Care & Collaboration area, including multiple new markets planned in 2018 and 2019. Our comprehensive approach to care includes extensivists managing acute and post-acute episodes of care, primary care clinicians, behavioral health clinicians, care management & engagement with case managers and community health workers, and mobile home-based care. The Extensivist, CareMore Primary Care, will serve as a hospitalist for a panel of complex Medicaid patients. The Extensivist is responsible for managing CareMore's population of patients through high-touch interactions and delivery of clinical services at the hospital, skilled nursing facility, and CareMore Care Centers. The Extensivist will report to the market Regional Medical Officer and will provide clinical oversight for a team of primary care advanced practice clinicians and care managers.

Essential Duties and Responsibilities

  • Creates a comprehensive and coordinated plan of care across all settings including hospitals, clinics, home and Skilled Nursing Facilities (SNFs), which take into account medical records, patient and family, primary care physicians, and specialists

  • Conducts daily hospital rounds

  • Collaborates with hospitals and care management teams and reviews patients with case managers

  • Attends to patients in CareMore Care Center (CCC) and assists nurse practitioners in reviewing cases

  • Attends to patients in the SNFs and attends SNF meetings to review cases

  • On call nights and weekends as scheduled


Minimum Qualifications

  • MD/DO; internal medicine or family medicine residency completion with board certification

  • At least 2 years' experience as a Hospitalist; or any combination of education and experience, which would provide an equivalent background

  • Current unrestricted medical licensure in relevant state and board certification

  • DEA license
