The Direct Care Program Manager provides management guidance
for the Division of Direct Care in the delivery of services to perinatal
clients in the community.  Duties
include the daily supervision of direct care services and staff.



 



Key Responsibilities: 



  • Manage the direct care services
  • Manage third party reimbursement services (Medicaid)
  • Develop and implement division polices and procedures  for the Direct Care Services
  • Provide management and coordination/integration of direct
    services with the Palmetto Healthy Start Dept. and other departments and /or
    perinatal care providers e.g. Palmetto Health Women's Clinic; Palmetto Health
    Hospital Discharge Planning Dept.; Richland Educational Dept. of Dental
    Services; March of Dimes Prematurity Prevention Grant
  • Develop and supply all required reports related to the
    performance and management for the division of direct care service.
  • Work with community development program manager and outreach
    staff as a community outreach team
  • Coordinate and provide training and continuing education for
    the Division of Direct Care
  • Coordinate direct care services with the community
    organizations that provide inter-conceptional care; dental services; perinatal
    health care; treatment for perinatal depression, etc.
  • Division of Direct Care Program Manager
  • Manage quality and productivity addressing all consumer
    complaints to assure satisfaction with services
  • Assist with monitoring of performance and evaluation of  all Direct Care Services within Palmetto Healthy
    Start
  • Manage the physical environment of direct care services to
    prevent injury and maintain safe conditions within the community setting i.e.
    enforce policies/procedures; education staff; report environmental dangers and
    follow up with action taken
  • Assist with meeting Healthy Start grant requirements,
    including reporting and preparation of various documents, e.g. continuation
    grant application
  • Develop and implement a quality improvement program through
    the consistent tracking of service objectives and support multi-disciplinary
    inter-department performance improvement activities (Time Study for
    Productivity Report)
  • Coordinate program with other health and human services providers
    (i.e. March of Dimes, Department of Health and Human Services Finance
    Commission, etc.)
  • Participate in development and implementation of Local
    Health Systems Action Plan
  • Perform other duties as assigned by Director


































 



Requirements:



Education: 
Bachelors Degree required; Masters Degree in Public Health, Nursing or
related field required 



     



Experience: Minimum of 5 years in community health
nursing supervisory experience preferred.



 



Licensure, Registry or Certification Required:
Registered Nurse Licensure in SC required or Licensed Practitioner of the
Healing Arts (LPHA)



 



Special Training: Healthy Start will provide any
special training for this position  
Physical Demands:  Must be able
to see, hear, communicate verbally and in writing and be mobile enough to travel
within a designated geographic area. Working Conditions:  Will work from Healthy Start office, field work
with staff, travel to meetings and conferences local, state, and national.  Must possess valid driver's license and use
of personal vehicle.
