My client operates both freestanding and "hospital-in-hospital" long term care hospitals designed to treat medically complex patients who require acute care hospital services for an extended period of time, generally more than 25 days. Currently, the company's network includes 20 locations in nine states. Their facility offers two convenient locations in the Nevada with over 50 total licensed beds on two campuses.
The Director of Case Management provides direction for the administration and ongoing development of Case Management operations, providing synergy with facility operations, providing supervision of case management staff and developing ongoing quality measurements for case management outcomes. The Director of Case Management is actively involved in department activities, transdisciplinary team activities, and my clients activities to ensure individualized, patient-centered health care for all patient populations admitted.
Will be responsible for training and maintaining clinical excellence, education budgeting, and identifying and targeting organizational needs in this area. The preferred candidate will have strong experience in critical care, preferably in the educator role and have appropriate certifications.
Required Education:
Graduation from an accredited technical school of nursing or master’s degree in social work.
If degree in nursing, BSN preferred
Required Certifications:
1. Currently licensed as a Social Worker or Registered Nurse (preferred) in state where currently practicing
2. Certified Case Manager preferred
Required Experience:
Minimum five years clinical case management experience and two years of management experience.
