Job Description:

Director of Case Management

The Director of Case Management has the responsibility to monitor, at the facility level, quality patient standards, patient and payor satisfaction levels, and for ensuring that patients are receiving prioritized, cost appropriate, and quality services.

Will assess, plan, organize, develop, implement and evaluate the case management program and activities in accordance with current federal, state and local standards, guidelines and regulations and JCAHO standards.

Demonstrate knowledge and skills necessary to provide case management services to the patient population ranging in age from 18 years to geriatric experiencing medically complex conditions, general medical-surgical conditions with rehabilitation and wound care needs. Supervision of case management staff.

An ideal candidate will have:
~ RN Degree from an accredited school of nursing.
~ CCM preferred
~ Must have current CPR, immunizations (MMR, DPT, Varicella, Hep B & TB screening)
~ 2+ years acute care hospital rehabilitation management or insurance-based case management experience.
~ Previous manager level and/or supervisory experience.
~ Clinical experience in an acute hospital or other related healthcare facility is preferred.
