As the Director of Case Management, you will:

* Effectively lead the Case Management Team

* Work integrally with the interdisciplinary team to coordinate plan of care

* Ensure proper and timely discharge planning for all patients

* Build and maintain effective relationships with physicians and referral sources

* Effectively promote Our Lady of Peace as the provider of choice

Our ideal candidate has:

* 2+ years experience case management

* Acute Care experience

* Active IN RN license

* BLS

* BSN preferred

* Certified Case Manager preferred
