Director of Case Management - LTAC Hospital

Vibra Specialty Hospital of Portland is the first hospital in Oregon to offer care for the medically complex patient for an extended stay. Our patients are admitted on a referral basis from local hospitals. We provide progressive care for our patients as part of a care plan that will ready them for rehab, skilled nursing, or home care. Our average length of stay is 25 days.

We are currently seeking an experienced Director of Case Management to join our team .  
 
The Director of Case Management has the responsibility to monitor, at the facility level, quality patient standards, patient and payor satisfaction levels, and for ensuring that patients are receiving prioritized, cost appropriate, and quality services.  Will assess, plan, organize, develop, implement and evaluate the case management program and activities in accordance with current federal, state and local standards, guidelines and regulations and JCAHO standards.  Demonstrate knowledge and skills necessary to provide case management services to the patient population ranging in age from 18 years to geriatric experiencing medically complex conditions, general medical-surgical conditions with rehabilitation and wound care needs. Supervision of case management staff.
 
 
Minimum Qualifications:
 
Minimum registered nursing degree from an accredited school of nursing.  Must have a minimum of two - three years acute care hospital rehabilitation management or insurance-based case management experience. Previous manager level and/or supervisory experience. Clinical experience in an acute hospital or other related healthcare facility is preferred.  CCM preferred. Current CPR,  and immunizations (MMR, DPT, Varicella, Hep B and TB screening) are required.

  • Demonstrates comprehensive knowledge of the principles and concepts of case management and applicable standards of voluntary and regulatory agencies for review activities.
  • Proficient in collecting and retrieving material from medical records.
  • Knowledgeable in utilization review, medical terminology, appropriate levels of care, treatment, modalities, statues, and healthcare delivery models.
  • Communication skills – both written and oral – must be outstanding.
  • Interpersonal relationship abilities must be highly refined with negotiation and problem solving skills at a very high level.
  • Must possess strong clinical assessment and process skills.
  • Strong organizational and analytical skills.
  • Ability to make independent decisions when circumstances warrant. 
  • Working knowledge of personal computer and software applications used in job functions (Word processing, graphics, databases, spreadsheets, etc.)
  • Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in this position.

  • Freedom from use of and effects of use of drugs and alcohol in the workplace.
Certificates, Licenses, Registrations
Current, unencumbered, active license to practice as a Registered Nurse in the state employed. CCM preferred.
 
Language Skills
English is the primary language of the facility.  Ability to read and write, communicate clearly with co-workers and customers and to respond to appropriate questions regarding work process or work product.
