Director of Patient Care Services
Yoh Health Care has a direct hire opening for a Director of Patient Care Services to join our customer in Tucson, AZ.
Responsibilities:
- The Director of Clinical Services is a critical member of the Touch team ensuring effective and efficient treatment of our members. This individual will be responsible for managing multiple facilities through the implementation of cohesive and efficient processes, with key emphases to include patient satisfaction, physician support systems and smooth operations. A strong understanding and ability to create metric to ensure the most effective administration of care is essential.
- Assist with developing, leading, hiring, teaching and mentoring all of the Touch Nurse Practitioner’s.
- Assist with growth and development of the Touch Program.
- Assist with development and management of the Touch clinical budget.
- Assist with Health Plan regulatory compliance.
- Assist with facilitating small and large group presentations, both with education and outreach.
- Performs physical examinations and preventive health measures within prescribed guidelines and instructions of the Physician.
- Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs.
- Records physical findings, and formulates a plan and prognosis, based on the patient’s condition.
- Discusses case with the Physician and other health professionals to prepare a comprehensive patient care plan.
- Submits the care plan and goals of individual patients for periodic review and evaluation by the Physician.
- Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
- Refers patients to the Physician for consultation or specialized health resources for treatment.
- Provides effective communication with facility staff, family members and clinical team to ensure implementation of the member’s care plan
