Key Competencies
Design data-driven interventions for all member guidance activities related to those at risk for, are newly diagnosed, or stable with chronic conditions. This includes all messaging activities to close Gaps in Care.
Develop identification and stratification tools to engage the appropriate members at the right time and determine the best program for them. This involves Health risk assessments, eHealth content, and Humana's Personal Health Record.
Receive, review, and prioritize ideas and concepts submitted to address at-risk or low-acuity chronic member needs.
Serve as the Product Owner to develop concepts through initial implementation, and then continuously improve them through regular review of predetermined performance metrics.
Collaborate with Operational and Market Leadership to transition initiatives from projects to business processes.
Develop measures of success, including targets for all initiatives and interventions.
Evaluate effectiveness of interventions in collaboration with the analytics teams.
Collaborate with other design directors (Acute and Chronic Complex) to coordinate interventions between domains.
Role Essentials
Bachelor's Degree in a clinically-related field.
5 years Managed Care experience in a leadership capacity.

You will have 3-5 direct reports, and you will report to a Vice President. This area is under the leadership of the SVP & Chief Operating Officer.
