Director of Social Services

Family owned Nursing and Rehab Center located in beautiful University City is seeking an experienced Director of Social Services with long term care experience. Our three person Social Services Department dramatically contributes to the quality of life and provides ongoing support to our 123 residents and their families. Our organization truly embraces the principles of Culture Change and resident empowerment. This is an exciting opportunity for a dedicated, hardworking, compassionate self-starter who is passionate about working with seniors. Experience in the care planning process and completion of MDS assessments required. Discharge planning experience desirable.
