Disease Manager
Tired of the clinic or hospital environment? PreferredOne has an excellent opportunity for an RN with outstanding clinical, interpersonal and computer skills who is interested in working in managed care, performing telephonic disease management of members with chronic conditions. You will provide education and support to members by speaking with them and their providers. The goal of this position is to assist members in maintaining good health despite the presence of a chronic disease.
Must be licensed as an RN in the state of MN with five years of clinical experience. This position will routinely require non-standard work hours (early evenings and Saturdays). Potential for future home based work.
