Job Description


 


Coordinates case management functions across the Mountain Division including but not limited to admission review, utilization review and discharge planning.  The Director will be responsible for recommending and/or implementing tools or changes (as needed) that ensure consistency, and maintain compliance in development, maintenance and system and process standards.  The director will support facility leadership to implement corporate case management initiatives.


 


KNOWLEDGE, SKILLS & ABILITIES:


§         Communication - communicates clearly and concisely, verbally and in writing.


§         Customer orientation - established and maintains long-term customer relationships, building trust and respect by consistently meeting and exceeding expectations.


§         Interpersonal skills - able to work effectively with other employees, patients and external parties.


§         Advanced PC skills - demonstrates knowledge and or proficiency in financial and clinical reporting systems.


§         Policies and Procedures - demonstrates knowledge and understanding of organizational policies, procedures and systems.


§         Basic skills - able to perform basic mathematical calculations, balance and reconcile figures, punctuate properly, spell correctly and transcribe accurately.


 


EXPERIENCE:



  • 5 years hospital case management experience required.
  • 2 years director/supervisor level experience

 


EDUCATION:



  • Bachelor  Degree Required

 


CERTIFICATE/LICENSE – RN Required
