The RN, Patient Care Manager, Hospice is responsible for the coordination of patient care for a specified patient care team. The PCM supervises the delivery of high quality, cost effective, compliant care to as many eligible patients as possible.
Principal Accountabilities 1. Coordinates all care rendered to patients and families. 2. Coordinates scheduling of team members to assure appropriate levels of care and to assure that all visits are completed. 3. Assures that staff follow documentation guidelines for accuracy, quality, content, and timeliness; assures that charts are properly maintained. 4. Assures that staff is familiar with appropriate policies, procedures and guidelines and is kept informed of changes. 5. Implements team building and staff support activities. 6. Responds promptly to any patient/family issues and takes appropriate action. 7. Coordinates IDG. 8. Serves as a member of the IDG. 9. As a member of the site leadership team, participates in providing orientation, education, leadership, coaching, accountability and mentoring to program staff. 10. Assures that patients are certified and re-certified appropriately. 11. Ensures compliance with all state and federal regulations. 12. Participates in Continuous Quality Improvement (CQI) activities and assures CQI reports are submitted in accordance with company policies. 13. Delivers direct patient care as necessary. 14. Coordinates on-call schedule to assure appropriate coverage and takes call. 15. Monitors patient care costs and staffing model to assure compliance with budget. 16. Responds promptly to regulatory surveys. 17. Assures that all outside services are properly authorized and bills are accurate. 18. Has a duty to detect, when reasonable, and report any potential misconduct or violation of VistaCare Standards of Conduct or federal or state laws and regulations. 19. Has a responsibility to participate in staff training and education programs and to ensure appropriate training of supervised staff. 20. Performs other duties as assigned. 21. (Texas only) Must complete six (6) clock hours per year of continuing education in subjects related to the duties of the administrator. 22. (Georgia only) As part of the leadership team, participates in establishing and implementing policies and procedures of routine nursing and personal care services according to accepted standards of practice.
Requirements
1. Meets all state regulations for Patient Care Manager. 2. Two (2) years of nursing experience; prior hospice experience preferred. 3. Two (2) years of clinical management experience, or demonstrated leadership ability within VistaCare. 4. Knowledge of business management and governmental regulations. 5. Bachelor’s or Master’s degree in nursing highly desirable. 6. Registered Nurse with current state license. 7. Highly organized, able to handle multiple and shifting assignments and present a professional image. 8. Demonstrated superior oral, written and interpersonal communication skills. 9. Strong team building skills. 10. Demonstrated computer skills including Microsoft Office, Internet and e-mail. 11. Reliable transportation, current driver’s license, and current auto insurance. 12. (Texas only) At least one (1) year experience as an RN obtained within the last 36 months
