DUTIES AND RESPONSIBILITIES:

 

• The registered nurse/field nurse (nurse) is responsible for providing Long Term Home Health Care Program (LTHHCP) patients with quality care in a safe and judicious manner.
  
• The nurse will assist, in conjunction with the Nurse Coordinator, in facilitating and coordinating all services that the LTHHCP Patients require.
  
• Responsible for promoting quality, patient access care, cost-effectiveness within a well integrated healthcare delivery system.
  
• Responsible for the on-going assessment, treatment and evaluation of the patients condition and family situation, as well as the supervision and evaluation of aide personnel that are assigned to provide services to the LTHHCP patients. 

 

EDUCATION/EXPERIENCE:

·                NY State RN license

·                NY Driver’s license and ability to provide own transportation

·                2+ years of recent experience working for CHHA or LTHHCP

·                Bi-lingual Spanish or Russian a plus

Salary up to $90,000 with excellent benefits
