HOSPICE CASE MANAGER RN

About the Job

This Medical Center is a 113-bed acute care; JCAHO-accredited hospital nestled in the beautiful and legendary Texas Hill Country. In fact, we’re perched on a hill of our own, overlooking the tranquil and historic area of the city.  We are centrally located to several water parks, outdoor malls, theaters, along the Guadalupe River that provides a venue for many water sports.


On September 4, 2008, (due to open November 2009) we broke ground on a $35 Million expansion project that will include a 2-story 64,000 square foot addition for a total of 139 private beds.   The expansion will create a new Surgery Suite with 6 operating rooms and 36 Medical/Surgical Cardiac-Monitored beds.  Another exciting component of the plan is a Women's Center that will feature 20 Obstetrical Rooms, 2  Ante-Partum Rooms, a Cesarean-Section Operating Suite, a High-Risk Delivery Suite, a Neonatal Critical Care Unit and 7 Labor/Delivery and Recovery Rooms.

When you or someone you love is diagnosed with a terminal illness; you are faced with many difficult decisions.  Our Medical Center Hospice is here to help. Our Hospice recognizes death as the final stage of life and is devoted to serving patients and families through compassionate, supportive, and holistic care.

We believe maintaining respect for dignity and individuality is essential to enhancing life. Our medical teams provide care in the comfort and familiarity you or your loved one's own home (or nursing home without invasive treatment or repeated trips to the hospital.


 


The hospice team develops a care plan the meets the patient’s individual needs for pain management and symptom relief.  The plan also outlines the medical and support services that will be required. This may include nursing care, personal care for the patient such dressing, bathing and basic hygiene, physician visits, social services and homemaker services. It also identifies the medical equipment, tests, procedures, medication and treatments necessary to provide high quality comfort care.


 


 


 


RN CASE MANAGER-HOSPICE


KEY RESPONSIBILITIES: The Hospice Case Manager is responsible for providing nursing care to hospice patients, in accordance with the physician’s orders and the interdisciplinary plan of care, with the goal of maintaining the comfort of the patient and family, and the highest quality of life that is possible, and in accordance with the standards of care and policies of our Hospice.


 


1.       Able to perform basic nursing functions in all aspects of care required in the Hospice setting.
 


2.       Ensures the comfort and control of symptoms of hospice patients by provision of nursing care, teaching and support of patients and families, preparation of the patients and families for the patient’s death, and by arranging for the provision of supplies, equipment, and medications needed for the patient’s comfort.


3.       Organizes and implements individual patient care based on ongoing interpretation of physiologic, psychological, and sociological needs which affect the comfort and quality of life of the patient and family.


4.       Plans and supervises the activities of the hospice aides as appropriate to individual patient requirements and in accordance with department policy.


5.       Works as a member of and cooperates with the interdisciplinary team members to provide comprehensive care and continuity of services.


6.       Communicates with the patient’s physician in matters pertaining to patient’s care, and adjusts the patient's plan of care as directed.
 


7.       Provides on-call coverage as needed.


8.      Maintains accurate, complete documentation of care in the clinical record in accordance with established agency standards and within established timeframes.


9.       Prepares and submits schedule planner of patient visits, and schedules visits with the Patient Care Coordinator.


10.   Participates in weekly Interdisciplinary Team Meetings and submits required documentation for the Plan of Care Update prior to meetings.


11.   Complies with department policies and procedures, practices, and standards of care.


12. Attends mandatory in-services and certification classes.


13.   Participates in quality activities including task force committee involvement as required.


14.   Demonstrates cost effective practices


15.   Responsible for insuring own clinical competence by maintaining professional and clinical skills through attendance at staff development meetings, pertinent educational activities, and professional affiliations. Keeps abreast of current trends by reading professional articles and journals.


16.   Completes related tasks as assigned by Hospice Clinical Manager/Director.


17. Serves as the leader of the patient care team.


 


18.   Prepares and submits schedule planner of patient visits, and schedules visits in conjunction with the Patient Care Coordinator.


Supervises the home health aide by evaluating compliance with the aide care plan and interview with the patient/family every two weeks in the patient’s home


 


EDUCATION AND EXPERIENCE


·         Graduate of an accredited school of Nursing


·         BSN preferred


·         Minimum of 1 year experience in Hospice Care


·         Must hold a current Texas RN license or be eligible for license as RN in state of Texas


·         Maintain Texas driver’s license with proof of liability insurance on file


·         Current CPR


·         Customer Service Attitude


·         Some Computer experience working with Healthcare Software desirable


·         Equipment knowledge: Home Medical Equipment, IV Pumps, Feeding Tube Pumps


·         This position reports to the Manager of Hospice Care.
