Description:
• Review health risk assessment (HRA) and identify any lifestyle risk(s) from the HRA.
• Assess covered individuals awareness of disease and initiate calls to assess member stage of readiness for change.
• Provide educational material and resource information to the disease management population by establishing the most appropriate way to support the covered individual.
• Assess member’s individual needs, conduct follow-up calls to monitor ongoing cases and to adjust education steps based on key interventions and clinical guidelines
• Review significant information and establish educational needs and goals or assess compliance with an established treatment plan
• Consults with Medical Director and clinical team for input into the Disease Management Process
• Maintains CEUs for current licensure
• Compiles and enters patient related data into established systems for tracking biometric information, achievement of goals, positive and negative outcomes.

Responsibilities:
• Ability to maintain patient/client confidentiality
• Familiarity with ICD-9 and CPT coding reference
• Bilingual skills (both written and verbal) preferable to effectively service our primary Member population
• Working knowledge of MS Office and McKesson Products

Qualifications:
• Certified Diabetes Educator with at least 3 years recent counseling experience, or
• Registered Dietician with at least 3 years recent diabetes and/or cardiovascular nutritional counseling experience

Certifications:
• Certified Diabetic Educator preferred, CCM eligible or CCM Certification preferred. CPUM. AHIP MMA or MHP certification.
• Registered Dietician (RD) with a current and valid licensure in the State of Texas, or
• CDE
