Making a difference since 1946, Cardinal McCloskey Services (CMS), is a not for profit human services agency providing abused and neglected children, at-risk families and developmentally disabled adults with the hope of a brighter future by providing millions of hours of services each year to 3,100 children and 2,000 families. Daily, CMS provides care to more than 1,500 children and their families and nearly 100 adults with mental retardation helping to enhance the quality of their lives and support the sanctity of families.


 


CMS is recruiting for a Health Care Integrator for our B2H program on 149th Street in the Bronx. Bridges to Health (B2H) is a state initiative designed to tailor comprehensive care plans for children in foster care with serious emotional disturbances, developmental disabilities, and medical frailties. B2H is the only program of its kind in the country and CMS is one of only five agencies chosen by the Office of Children and Families' Services to participate.


 


The Health Care Integrator (HCI) is expected to assess, educate, coordinate, consult, stabilize, advocate, and provide linkage services for children enrolled in the B2H waiver program and, as such, is the primary architect of the child's Individualized Health Plan (IHP). HCI's have primary responsibility for the child's health coordination and administration of the IHP.


 


Responsibilities of the position include:


- Assessing the need for services and developing the IHP


- Link the child and family/caregiver to the identified services


- Consult with the child and caregiver on the appropriate provision of services


- Stabilize the enviornment for the child and caregiver, and advocate for the child's needs


- Coordinate the constellation of services and providers to meet specific needs of each enrolled child


- Participate in overall quality management of B2H Waiver Program


 


Successful candidates will have a masters degree in psychology or social service related degree. Previous experience with special needs children is preferred. Bilingual (English/Spanish) is preferred. This position requires home visits.
