At Compassionate Home Health Care our focus is to provide skilled care to promote independence and optimal physical and emotional health to remain at home during the healing process. We believe in providing compassionate care to all patients and are strongly committed to the needs of our patients and their families/caregivers. On top of our skilled specialty services, we provide a Nurse on call 24 hours a day, 7 days per week for any and all patient needs.

We are currently seeking licensed professionals for direct hire part-time and contract positions with an area of coverage that includes Oakland, Wayne, and Livingston Counties.

v  Physical Therapist (PT)

o   Performs physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation for clients who are receiving services under a medically approved Plan of Care.

o   Develops a therapy care plan to provide direction to other health team members involved in the plan of care. Reviews and updates per Agency policy and client need.

o   Documents all findings, plans, interventions, and outcomes.

v  Occupational Therapists (OT)

o   Performs occupational therapy assessments, diagnostic tests, and skilled treatments.

o   Develops a treatment program and establishes goals for improved function.

o   Documents all findings, plans, interventions, and outcomes.

v  Registered Nurse (RN)

o   Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.

o   Completes comprehensive assessments including OASIS data set.

o   Develops and/or follows an individualized Plan of Care.

o   Documents all findings, plans, interventions, and outcomes.

v  Speech Therapists (SLP)

o   Performs speech therapy assessments, diagnostic tests, skilled treatments and ongoing evaluation.

o   Develops a speech therapy plan that identifies functional speech defects or swallowing difficulty and is directed toward improving client function.

o   Implements teaching appropriate to the client's needs and abilities, and evaluates the effectiveness of the plan while providing other team members with guidelines that support the client goals.

All roles require a minimum of one year adult home care experience as well as experience working in assisted living facilities and home settings. Experience with OASIS required for PT, OT, and RN positions.

All positions are pay per visit (including mileage) and are done at either an upscale Senior community or the patient’s home.

All candidates must have the appropriate professional licensing for Michigan and a valid driver’s license.
