VMT Home Health Agency is seeking an experienced nurse with Home Health knowledge.  Must be familiar with OASIS and coding. In addition to the above, listed below are additional responsibilities.

Candidate must be able to:

Ø      Conduct patient visits weekly and monthly

Ø      Ascertains that all referrals for Home Health Services are responded to in a timely manner

Ø      Maintains efficient and effective operation of Nursing Services

Ø      Represents Home Health Agency in a professional and competent manner

Ø      Promotes Home Health Services through marketing efforts

Ø      Insurers compliance with federal, state and JCAHO regulations/standards

Ø      Participates in strategic planning process

Ø      Functions as clinical resource to Home Health Services staff

Ø      Ascertains all clinical notes are appropriately documented that supports billing function

Ø      Insures physician order are carried out

Ø      Ascertains Plans of Treatment are appropriate and revised as needed

Ø      Ensures timeliness and completeness of recertifications

Ø      Ensures completion of chart audits as required i.e.: quarterly record and utilization reviews

Ø      Cooperates with other departments to maximize service delivery opportunities

Ø      Serves on Committees and Professional Advisory Board as appointed by supervisor

Ø      Develops and maintains constructive and harmonious working relations with co-workers, clients and referral sources

Ø      Is knowledgeable and supportive of VMT Home Health philosophies, polices and procedures. Utilizes appropriate VMT operations manuals as references for operational questions

Ø      Meets with supervisor on a regular basis to discuss problems, issues and opportunities to improve services

Ø      Resolves complaints from clients and/or referral sources

Ø      Conducts Home Health Services quality assessment/quality improvement activities

Ø      Conduct in-service trainings as needed for nurses and home health aides on various topics

Ø      Maintains current knowledge of reimbursement issues
