Hospice Nurse-Case Manager (20091139)

Job Duties Include: A Registered Nurse is a licensed professional who provides direct and indirect patient care services under a physician's plan of care and in accordance with the policies and procedures of the Walgreens-Option Care, the state's professional standards, and regulatory entities. The Registered Nurse is responsible for assessment of all patients assigned to him/her, establishing and implementing the plan of care plan, educating patients/caregivers, evaluating progress toward goals and making recommendations to the physician for changes in the plan of care based on patients needs.

Minimum Requirements:

• Graduate of an accredited school of nursing with valid license in state(s) of practice.
• Valid and current driver's license and evidence of automobile insurance.
• Current CPR certification.
• Minimum of one year of experience as a registered nurse.
• Flexibility and the ability to work effectively within a multidisciplinary team.
• Ability to communicate effectively (verbal and oral) with team members, patients, caregivers, physicians and referral sources.

Preferred Qualifications:

Previous Home Health Care and previous Infusion Experience. BSN preferred but not required.
