You’ll be part of a professional team that will provide support which includes assessment, planning, and care of our clients and their family members in the comfort of their home.   

We’re looking for a full-time Intake Coordinator to travel within and around the Boston Region*.  This area encompasses towns between Winthrop, Lexington, Dedham, South Boston and Boston Proper. 

The Position:  You’ll be the first point of communication and contact to potential clients of our program via telephone triage and the internet.  By providing general program information you’ll be able to assist with matching the caller profile with eligibility requirements.  Once placed, you’ll complete the initial home assessment and caregiver credentialing activities.  By using our web-based case record system you’ll be able to provide timely and accurate documentation as well as conduct follow-up calls as necessary.   

Requirements:  Bilingual (Spanish/English); Bachelor Degree in Social Work, Nursing or other related field, or an equivalent combination of education and experience in these related fields.   Minimum of two years experience in social service, healthcare or related field, with knowledge of eldercare and disability services and programs in the community and across the continuum of care.  You should possess the ability to work effectively in a fast-paced environment, with a keen attention to detail, and the ability to work remotely as field-based staff.  You must be computer proficient with knowledge of MSO suite.
