Intake Nurse

Position Summary : Intake Nurse is responsible for clinical evaluation of all new PAH referrals as well as telephonic clinical assessment of the patient. Intake nurse reviews referral and criteria for appropriateness of therapy. RN must have excellent communication skills to communicate with internal staff, MDs, as well as patients. Also identifies and coordinates care with home health agency nurses when necessary.

To be considered for this position, candidates must have graduated from an accredited nursing program, be a Registered Nurse with a current nursing license in the state of Illinois and have a minimum of 5 years recent clinical experience with 2+ years of home infusion therapy experience. Candidates must have extensive knowledge of and experience with infusion therapy supplies and pumps; demonstrate effective problem solving ability and have initiative to work independently in managing the delivery of participant care and as part of a coordinated team. Excellent interpersonal, verbal and written communication, time management, organizational and customer service skills are required along with the ability to be licensed in any other state if needed.
