Intensive Care Coordinator
Intensive Care Coordinator Justice Resource Institute’s (JRI) Dimock Community Service Agency (CSA), located in Jamaica Plain is accepting full-time applicants for a Children’s Behavioral Health Initiative (CBHI) Intensive Care Coordinator (ICC) position. Intensive Care Coordination (ICC) is a service that facilitates care planning and coordination of services for eligible youth with a serious emotional disturbance under the age of 21. Care planning is driven by the needs of the youth and their family and developed through a Wraparound planning process. Responsibilities include but are not limited to: - Coordination and care planning of multiple services/supports for families with a child/youth with serious emotional/behavioral health needs.
- Managing individual caseload.
- Meeting all productivity standards.
- Timely submission of all required paperwork such as CANS, comprehensive assessments, individualized care plan, daily progress notes and insurance authorizations.
The minimum staff qualifications for a Senior Care Coordinator include: - Masters Degree in social work or other mental health discipline required.
- Experience in working with children/families with serious emotional/behavioral health needs.
- Must have knowledge of Wraparound systems of care and child welfare.
- Must have excellent communication, organization, and time management skills.
- Must have experience working collaboratively with state agencies, consumer advocacy groups, and/or behavioral health outpatient facilities.
- Must be certified in the Massachusetts CANS.
- Applicants must have a valid driver’s license and reliable transportation.
