I. TITLE: HABILITATION COORDINATOR CODE:

II. SUMMARY: An employee in this classification, under the supervision of the Adult Options Manager, is responsible for the supervision and monitoring of Adult Options habilitation staff, Medicaid billing documentation, and day program services.
III. SUPERVISORY RESPONSIBILITIES: Supervises and orients individuals assigned to the department.
IV. DUTIES:
40% Responsible for the supervision, evaluation, training, support and coordination of staff including interns and programs. Conduct investigations and administrative follow-ups including disciplinary actions as needed. Attends and/or conducts meetings, conferences and seminars. Daily review of all unusual incidents and Major Unusual Incidents to insure appropriate follow-up. Monitors activities to assure compliance with all applicable rules, regulations, laws, practices, and collective bargaining agreements.
20% Program Monitoring: Review all Individual Service Plans (ISP) and Individual Plan (IP) documents to ensure accuracy and proper representation of Self determined initiatives, directions and choices made by individual served and their families; Coordinates movement of individuals served into/out of and within the facility; Coordinate, where appropriate, vocational programs/services with other departments as well as other community agencies (i.e. school districts, adult services providers, Rehabilitation Services Commission, residential service providers); and Responsible for the facility and its operation during the absence of the Adult Options Manager.
20% Behavior Support and Health and Safety: Ensures positive support for Individuals in crisis to protect health and safety; Attends Behavior Support Committee meetings to ensure coordination of Behavior Support Plans; Responsible for training and development of staff and support team(s) to ensure appropriate positive behavior support and crisis intervention; Develops, coordinates, train and support the facility behavior support team; and Ensure appropriate follow-up, training, counseling and other administrative actions.
15% Tracks and Monitors Medicaid Waiver Billing documentation: Monitor and track daily empower time sheet and time card information; Provide support and corrections for billing documentations for accurate billing for services; and Provide daily monitoring for acuity report.
5% Performs other related duties as required. Required to transport persons served and/or drive an agency vehicle. Habilitation Coordinator
V. EQUIPMENT OPERATED: Personal computer, applications software and basic office equipment.
VI. WORKING CONDITIONS: Position may involve exposure to individuals with medical and behavioral risks.
VII. KNOWLEDGE, ABILITIES AND SKILLS: Knowledge of habilitation and related professional services practices and procedures. Knowledge of local, state and federal laws, rules and regulations, involving habilitation and other related programs. Ability to use sound judgment in dealing with a variety of situations. Ability to establish and maintain harmonious relationships. Effective organizational and responsive communication skills. Adherence to the principles of good management practices as outlined in the "Administrative Team Expectations of Effective Leadership and Management Practices".
VIII. MINIMUM QUALIFICATIONS, EDUCATION AND EXPERIENCES:
A. Education: Master degree in Vocational Rehabilitation, Education, Counseling, Psychology or related field.
B. Experience: Three (3) years experience working with individuals with developmental disabilities. One (1) year of supervisory experience preferred.
C. Credentials:
1. Certification: Certification as issued by the Ohio Department of Developmental Disabilities.
2. Registration: N/A
3. Licensure: Valid driver’s license.
D. Ancillary Training:
a. Standard First Aid/CPR
b. Specialized Supports Resource Book Training
E. Other: Requires own transportation. Will require evenings, weekends, and locations convenient for individual/family served by the Board.
