Position Details:
Demonstrate basic knowledge of pharmacology and medication administration techniques per facility protocols, and state regulations, as prescribe by contract physician.
Observe patients and report adverse reactions to medications or treatments.
Perform cardiopulmonary resuscitation (CPR) and assist during respiratory and cardiac arrest procedures.
Perform a range of nursing procedures, including dressing changes, suture removal, oxygen administration, wound irrigation, catheterization and venipunctures.
Prepare patients for laboratory procedures and examinations by providing information and instructions, while assisting the PCP and RN Manager upon request.
Set up and operate therapeutic and emergency equipment, including monitor, EKG, oxygen and portable suction.
Document all pertinent patient information in the patient’s medical records and VISTA to demonstrate quality care delivery and promote continuity of care.
Assist in maintaining the VISTA (Veteran Health Information Systems and Technology Architecture) clinic reminder tracking system for each enrolled patient.
Assist in scheduling patients for initial and follow‑up appointments in accordance to the VA access standards and the patient’s medical necessity.
Assure that patients are informed regarding their plan of care, including lab and diagnostic test results as needed.
Provide health educational, materials and resources to patients and their families for informational purposes.
Participate in the ongoing Performance Improvement Program (PIP) between Health Net, its Subcontractor(s), and the Veterans Affairs Medical Center (VAMC).
Assist in ensuring that all required reports are completed in an accurate and complete fashion.
Assist the RN Manager in maintaining the clinic compliance with all Federal, state, local, JCAHO, OSHA, Veteran Affairs, Health Net VA Program and Subcontractor safety and operational regulations, directives and standards.
Ensure all patient information on encounter forms is completed and accurate, in conjunction with the CBOC staff.
Maintain confidentiality of all information and support patients’ privacy, rights, and safety.
Act as a liaison between Health Net and the VAMC as related to patient coordination/continuity of care related issues.
Perform other work‑related duties as assigned.
Experience/Qualifications Needed:
LVN/LPN from an NLN-accredited school.
Retention of current nursing license in the state in which the LVN/LPN practices.
Minimum of 2-3 years Medical-Surgical, ICU, and/or Emergency Room (ER) experience preferred.
Retention of current CPR/BLS.
Knowledge of and ability to apply professional nursing principles, procedures and techniques.
Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations.
Emotional stability with a caring, sympathetic nature.
Ability to follow orders and work under close supervision, while demonstrating initiative and independence.
Ability to provide, as directed by the physician, individualized and group counseling, guidance, and health instruction with the focus on disease prevention and health maintenance.
Ability to teach patients and/or families proper home health care activities, such as giving injections, taking blood pressures, changing wound dressings, etc.
Knowledge of pharmacological agents used in patient treatment, including desired effects, side effects, complications, and patient usage considerations.
Knowledge of the basic concepts of customer service techniques related to age-specific population.
Demonstrated effective verbal and written communication skills along with proper telephone etiquette.
Basic knowledge of computers.
Hours:
Monday through Friday, day shift.
