Duties:

LPNs teach and empower patients to independently perform their medical care in the comfort of their home. Clinical care ranges from post surgical care of trauma, orthopedic, cardiac and general surgery conditions. The LPN case manager provides management of chronic diseases such as diabetes, cardiac and respiratory diseases.

Qualifications:

Current unrestricted LPN license in the state of Virginia.
Two years of current Home Health care experience, two years of recent acute care experience, or two years of recent hospice experience

Current CPR License
Valid driver’s license, and auto insurance
Proficient computer skills & electronic documentation experience preferred
Excellent customer service skills experience
