Medical Social Worker

Provides a wide range of psychosocial services to terminally ill patients and their families, including regularly assessing the patient and family psychosocial needs, and Pre-Bereavement/anticipatory grief needs. Implements the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care. Regularly re-evaluates the patients and familys psychosocial needs

Job Requirements
Odyssey's Ideal Candidate will have the following qualifications:

  • Minimum age requirement of 18
  • Bachelor's degree in Social Work, Must have Licensed Master's degree from school accredited by the Council on Social Work Education required in states as specified
  • Licensure and/or certification as required by the State.
  • Minimum of two year?s social service experience in a health care setting or social services agency
  • Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.)
  • Compliance with accepted professional standards and practices
  • Understands Hospice philosophy, principles of death/dying
  • Ability to work in an interdisciplinary setting
  • Valid driver's license and an automobile that is insured in accordance with state and/or Odyssey requirements and is in good working order
  • Demonstrate excellent observation, judgment and communication skills. Ability to provide good written documentation in a timely manner
  • Self-directed with the ability to work with little supervision
