CareMore's Primary Care & Collaboration division oversees strategy, operations, and care delivery in our primary care markets (Iowa, Tennessee, Connecticut), where CareMore builds and runs capitated primary care medical groups and currently serves over 22,000 high-risk Medicaid and Medicare patients, as well as our collaboration with Emory Health System, which brings the CareMore model to over 20,000 Medicare Advantage patients in Georgia. The division also supports the development of new markets and models in the Primary Care & Collaboration area, including multiple new markets planned in 2018 and 2019. Our comprehensive approach to care includes extensivists managing acute and post-acute episodes of care, primary care clinicians, behavioral health clinicians, care management & engagement with case managers and community health workers, and mobile home-based care. The Manager, Community Engagement CareMore Primary Care is responsible for establishing CareMore's relationship with the Fort Worth, TX social service community. This individual will be integral in driving patient engagement and establishing connections between CareMore and Fort Worth, TX community centers, non-profits, government agencies, etc. He/She will be responsible for managing a team of community health workers, who directly engage with complex, Medicaid patients, facilitates patient appointment scheduling and ensure that the population's personal and social needs are met.

  • Ability to build partnerships with local community organizations and serve as a key representative of CareMore in the local market

Essential Duties and Responsibilities

  • Manages a team of Community Health Workers and works closely with Case Management team to provide a coordinated care delivery approach for patients
  • Ensures appropriate Community Health Worker patient paneling and coordination with CareMore clinical team.
  • Spearheads patient engagement -- manages both the list of patients prime for enrollment as well as the process by which the CareMore team initially engages and retains patients
  • Utilizes standardized tools for tracking patient engagement and community outreach.
  • Develops monthly reports for GM/RMO
  • Identifies, develops and builds relationships with community organizations, associations, emergency services (Police/Fire/Ambulance), associate provider offices, and faith based organizations; performs activities to enhance presence in the community and develops collaborative opportunities
  • Leads continuous grassroots engagement and marketing strategy for CareMore in the Fort Worth, TX area
  • Builds and trains on a market Community Engagement playbook that focuses on guiding direct reports and future hires on how to both assess and alleviate social, cultural or financial problems using local Fort Worth resources
  • Partnering with the health plan marketing team on outreach events, public relation opportunities, etc..
  • Engaging the CareMore Marketing team and developing collateral material, coordinating media events annually, and ensuring promotional materials are available.
  • Collaborating with CareMore leadership team on the development and deployment of the annual outreach strategy.
  • This position requires after-hours (for events past 5:00 p.m.) as well as weekend events on Saturday and Sunday.
  • Requires BA/BS degree in marketing, public health, health education or other related discipline,
  • 5 years related experience, or any combination of education and experience, which would provide an equivalent background.
  • Requires exceptional networking & negotiations skills, and strong public speaking/presentation skill, and previous consumer marketing and/or grassroots/community outreach experience.
  • 3 years of experience in case management in a healthcare environment; or any combination of education and experience, which would provide an equivalent background is highly preferred.
  • Licensed Independent Clinical Social Worker (LICSW) or Licensed Independent Social Worker (LISW) a huge plus/highly preferred.
  • Current unrestricted LSW license in applicable state(s) a huge plus/highly preferred!
  • Bilingual - Spanish a plus
