Manager of Clinical Practice - Conyers, GASUMMARY: Manages the development/implementation of care plans within scope of practice, consults with third party representatives regarding patient financial responsibility, and interfaces with Case Managers to obtain reauthorizations for additional services and supplies. Implements standards, maintains the integrity of operational policies, participates in performance improvement activities as appropriate. Works under general direction. ESSENTIAL DUTIES AND RESPONSIBILITIES: Conducts/delegates the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Company, physician, and/or health facility procedures/policies. Manages the assignment of caregivers. Coordinates communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning and ensures compliance with third party requirements through review of documentation and care coordination activities. Interprets standards/company policies/procedures to ensure compliance with external regulatory authorities and ensures that caregiver clinical documentation meets internal standards. Consults with third party representatives regarding patient financial responsibility, service authorization and reimbursement procedures. Negotiates reimbursement levels and facilitates the process. Participates in the recruitment/interviewing/selection/orientation of team members, evaluates their performance relative to job goals/requirements, coaches staff recommending in-service education programs and ensures adherence to internal policies/standards. Participates in performance improvement activities, maintains ongoing clinical knowledge through internal/external training programs, provides interpretation of knowledge and direction to staff. Maintains relationships with referral/community sources, participates in professional organizations and conducts care related programs. Performs other duties as assigned. QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE: Associate's Degree or Diploma in Nursing or the equivalent or a Graduation Degree from an accredited Physical Therapy Program (approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association and/or the American Physical Therapy Association) and meet any additional qualifications as stated in state home care regulations with a minimum of three years experience in area of specialty preferably in a community or home care setting with one year in a supervisory role generally required. Licensure in the state(s) of practice required. Excellent organization, problem solving and communication skills also required. |
